Library Card Application Form

We would love to have you as a member of our library! In order to receive a library card, you will need to complete the two forms below. Be sure to click register on the first form and click submit after going through all of the pages on the second form. Both forms are required. Once you have filled out the forms, and read all of the information in the forms, stop by the library with a valid ID and proof of residence to pick up your library card. It's that easy!

Frequently Asked Questions

What types of ID do we accept?
Any State or Federal ID (If you’re moving in from out of state, don’t worry. It doesn’t have to be from the state of Utah. We just need to see that it’s you.)
School IDs
Passports
Veteran or Military IDs
North Logan City or Public School Employee ID
Public Assistance ID

What Counts as Proof of Address?
A document with your name as it appears on your ID and your address. This includes mortgage or lease documents, bank statements, bills, insurance papers, mail from a government entity, etc. We accept paper statements and electronic statements.

Can Kids Get Their Own Cards?
Yes! Children over 5 can get cards with a parent or guardian’s permission. A parent or guardian will need to give permission in the online library card application form. They will also need to show ID and give permission in person in the library.

Can Other Members of My Family Use My Card?
Yes, you can name specific people as users of your card. Let us know at the desk and we can add authorized users to your account.

Can I Get a Card If I Don’t Live in North Logan?
Yes. We have nonresident cards available for a small fee.
    3 Month Membership-- $24 per family (great for Summer Citizens)
    1 Year Membership--$90 per family

I Live in Hyde Park. Do I Have to Pay for a Card?
Yes. Because North Logan city taxes fund the library, only North Logan residents can obtain a resident card.

What if I work in North Logan, but don’t live there?
If you work for an organization funded by property taxes in the City of North Logan, such as North Logan City, or any North Logan public school, you can still get a card. Just bring in the ID, or proof of employment, associated with your North Logan City or school employment to verify that you are a current employee.

You can also get a card if you own a business in North Logan. Just bring in proof that your business operates in North Logan.

How Long will my Library Card Remain Active?
Resident cards are good for 3 years. To renew your card, we’ll ask you to bring in proof of address again, and make sure your contact information is up to date.
Nonresident cards must be renewed quarterly or yearly as specified above.
